Di Malaysia CRC adalah kanser yang kedua terbanyak, malangnya kebanyakan kes kanser iaitu 65% dikesan di tahap III dan IV.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Kekurangan vitamin D adalah di antara faktor punca penyakit ini dan pemberian vitamin D secara suplemen membantu penyembuhan kanser kolonorektal. (1,2,3)<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Banyak kajian yang dilakukan dalam makmal menunjukkan bahawa vitamin D menyebabkan sel-sel kanser berhenti membiak dan lebih mudah mati (<strong><em>apoptosis<\/em><\/strong>), pengubahsuaian perjalanan <strong><em>Wnt signalling pathway<\/em><\/strong> (kitar hidup sel dari mula dibentuk sehingga kematiannya), pembaikan DNA dan mempertingkatan pertahan diri <strong><em>(immunomodulation<\/em><\/strong>). (4). Ini mengukuhkan bukti bahawa vitamin D adalah penting dalam menghalang dan merawat kanser konorektal. (5)<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Selain itu kanser kolonorektal juga berpunca dari obesity, masalah pengambilan daging merah yang banyak terutama yang dibakar, kurang mengambil serat dalam pemakanan. Faktor genetic (keturunan) juga memainkan peranan yang besar dalam punca penyakit ini.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Dalam beberapa kajian gagal menunjukkan kepentingan vitamin D dalam mengelakkan kanser kolonorektal (6) tetapi kajian ini mendapat kritikan kerana sukarelawan yang terlibat dalam kajian itu sudah pun mempunyai tahap vitamin D yang mencukupi dalam tubuh. Jadi pemberian vitamin D pada sukarelawan yang sudah mempunyai tahap vitamin D yang mencukupi dalam tubuh sudah tentu tidak mengubah apa-apa keputusan yang diterima. (9)<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Dalam kajian yang menyokong pula membuktikan tindakbalas antara genetic yang berkait dengan vitamin-D, tahap vitamin D <em>25-hydroxyvitamin D (25OHD)<\/em> dan pembentukan sel kanser kolonorektal. Terdapat penurunan pada kadar kematian akibat kanser kolonorektal setelah kes-kes mereka dilakukan follow up selama 2 tahun. (7,8)<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Dalam kajian yang diterbitkan dalam <strong><em>British Journal of Cancer, 2018<\/em><\/strong> pemberian suplemen vitamin D terhadap pesakit kanser dapat menurunkan kadar kematian sebanyak 35% pesakit kanser. Kajian ini melibatkan 65 000 pesakit. (9) Juga terdapat kajian yang menunjukkan tahap vitamin D yang rendah sangat berkait rapat dengan <em>survival<\/em> (kelangsungan untuk terus hidup) pesakit kanser yang rendah. (10,11).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Dos yang dicadangkan berdasarkan kajian SUNSHINE bahawa 4000IU, dan bukannya 400IU kerana dos 400IU dianggap tidak mencukupi. Dos yang rendah ini adalah dos yang memastikan tahap vitamin C dalam tubuh \u226510\u2009ng\/L dengan syarat seseorang individu itu mendapat sinaran matahari setiap hari yang mencukupi. (12). Malah dos sebanyak 2000\u20134000\u2009IU\/day bagi orang dewasa dan seharusnya menjadi piawaian bagi kajian akan datang. Dikatakan pengambilan dos yang rendah akan menyebabkan tiada kesan yang positif dalam kajian yang yang dilakukan. (13,14,15,16)<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Secara kesimpulannya, pengambilan vitamin D 2000-4000 IU adalah sangat bermanfaat bagi pesakit kanser konorektal dan bagi mereka yang berisiko dan ingin emngelakkan diri dari penyakit ini.
